If you right click somewhere in simba, then wait for the 'Smart mouse' to click somewhere in runescape, Smart will no longer take focus away from Simba. Leaves you free to write scripts while botting ;]
EDIT: seems like it also works if you switch away from both smart and simba, and then wait till smart clicks in rs.
So as long as smart does not have focus when SMART clicks its all good
