This would take forever for me to just sit here and convert. Just learn how to read music and you can write it in yourself :)
The first chord in the right hand is E on top, C in the middle, then G on bottom (C chord 2nd inversion).
The first note in the left hand is a C. Just base everything off those notes then. abcdefg. So if a note is 3 higher then the C in the left hand, and it has a sharp sign, then it is an F sharp. If a note is one above that, then it's a G.
Writing in the notes for music really isn't hard at all if you have a reference point. Just count from a certain note. Just remember once you get to G, the next note is an A. The hard part about reading music is being able to convert the note to the key on the piano quick.
If you want any help you can add me on msn: runescapemacro@hotmail.com I took piano lessons for 8 years and still play today, so I'll hopefully be able to help.

Just do it yourself
Treble clef (top staff)

Bass clef (bottom staff)

I have my old music theory 1 book where it basically teaches you how to read music through chord progressions etc.
I might be willing enough to scan it for you to learn, although its like 120 pages of workbook so its a lot of scanning.
