What makes something loading RS a client?

I mean, Any program could be called a client ie. Simba.

A client is a program that is not stand alone, but instead relies on a server to function. The runescape client doesn't contain the entire game. Just what is needed for communication and displaying the world, the server handles the other part of the game.
