Starting a new script and I'm trying to make it fancy, havent done a form before so I need some help, i keep getting the error

Error: Exception: A control can't have itself as a parent at line 74
im following a couple guides so maybe i mixed something i wasnt suppose to?
Any ideas?
I'm learning so idk where the text's are going show up, i know how it works but i just put some numbers in :P
